Lost or Stolen Passport

Americans should safeguard passports as they would any other valuable.  If your passport is lost or stolen in Azerbaijan, come to the Consular Section to report the loss and apply for a replacement.

Adults Must:

• Come to the Embassy in person

• Complete forms DS-11 Application for Passport and DS-64 Statement Regarding Lost or Stolen Passport.  You may download the forms or obtain a copy at the Consular Section.  Please do NOT sign the forms until the Consular Officer instructs you to do so.

• Present proof of U.S. citizenship by providing any one of the following:

• Certified copy of birth certificate
• Certificate or Report of Birth Abroad
• Certificate of Naturalization
• Certificate of Citizenship

• Present one current valid form of photo identification to prove identity (e.g. foreign passport, driver’s license, military ID, etc.)

• Provide two passport photos

• If you are resident and employed in Azerbaijan, you may also bring a letter confirming that you reported the loss to your company.

NOTE: All applicants wishing to replace a lost or stolen passport must appear in person

Approximate Processing Time:  2 weeks.
Fee: $100.00.

Minors Under the Age of 16 Must:

• Present Evidence of Child’s Relationship to Parents/Guardians by submitting one of the following  with parents’ names:


• Certified U.S. birth certificate
• Certified Foreign Birth Certificate (translation, if necessary)
• Report of Birth Abroad (Form FS-240)
• Certification of Birth Abroad (Form DS-1350)
• Adoption Decree (with adopting parents’ names)
• Court Order Establishing Custody
• Court Order Establishing Guardianship

• Present one current valid form of photo identification to prove identity of each parent (e.g. foreign passport, driver’s license, military ID, etc.)

• Both parents must appear together and sign the application or one parent may appear with either 1) notarized documentation of the other parent’s consent to issue a passport to the child or 2) documentation of sole authority to apply

• Provide two passport photos

NOTE: All applicants wishing to replace a lost or stolen passport must appear in person

Approximate Processing Time:  2 weeks.

Fee: $85.00.
