To apply for a U.S. visa you must:

1. COMPLETE AN ELECTRONIC VISA APPLICATION FORM DS-156

Complete and print an Electronic Visa Application Form (DS-156) in English. This form MUST be filled out electronically.  Once you have entered your information, click "CONTINUE" to print the completed application.  Click here for instructions in Azeri or Русский. Handwritten or typed applications are not accepted.  

2. MAKE AN ONLINE APPOINTMENT

The consular section does not accept appointment requests via the telephone.  Applicants must make appointments via our online appointment system.  Once you fill out the application form online, you have to use the barcode number on the 3rd page of your application to schedule an appointment. 3. ATTACH NECESSARY DOCUMENTS REQUIRED

- Valid Passport: a passport valid for travel to the United States and valid for at least six months beyond the date the applicant will exit the United States.

- One photograph: a photograph, 5cm X 5cm on a white background, taken within the last six months.

- Application Form DS-157: Only for male applicants between the ages of 16 and 45

- Application Form DS-158: Only for students and exchange visitors

- Visa Application Processing Fee $131 payable in U.S. dollars or the equivalent in new Azerbaijani manats. This fee is non-refundable

4. COME TO THE EMBASSY to apply for your visa

Consular Section of the U.S. Embassy, Baku is located at 83, Azadlig Prospect. We accept visa applications 4 days a week: Monday, Tuesday, Wednesday, and Thursday, between 9:00 a.m. and 12:00 noon, only by prior online appointment at an assigned time.

Applicants must submit the relevant application forms, passport and supporting documents, and pay the non-refundable $131 visa application processing fee at the time of their appointment. The applicants will be interviewed the same day. 

Immediately upon the conclusion of the interview, each applicant will be informed whether a visa will be approved and, if so, when it will be ready for collection. Visas may be processed within 24 hours, and wherever possible will be issued on the next day.
