Correcting Passport Information

If you change your name or find a printing error in your passport please follow the instructions below to correct the information in your passport.

Name Change

If you change your name WITHIN one year from the date of issue of your currently valid U.S. passport, you must:

• Come to the Embassy with the current, valid passport containing your previous name
• Complete form DS-5504 U.S. Passport Re-Application.  You may download the form or obtain a copy at the Consular Section.
• Submit acceptable evidence of name-change, such as marriage/divorce/adoption decree; or court order of name change
• Provide two passport photos

Approximate Processing Time:  2 weeks.

Fee:  There is no fee for this service.

If you change your name MORE THAN one year from the date of issue of your currently valid U.S. passport, you must:

• Come to the Embassy with the current, valid passport containing your previous name
• Complete form DS-82 Application For Passport by Mail if you are eligible, otherwise complete form DS-11 Application for Passport.  You may download the form or obtain a copy at the Consular Section.  Please do NOT sign the form until the Consular Officer instructs you to do so.
• Submit acceptable evidence of name-change, such as marriage/divorce/adoption decree; or court order of name change
• Provide two passport photos

Approximate Processing Time:  2 weeks.

Fee:   $75.00 for adults.
         $ 85.00 for children.

Printing Errors

If you discover a printing error in your VALID passport, you must:

• Come to the Embassy with the current, valid passport containing the error
• Complete form DS-5504 U.S. Passport Re-Application.  You may download the form or obtain a copy at the Consular Section.
• Submit acceptable evidence showing the correct information
• Provide two passport photos

Approximate Processing Time:  2 weeks.

Fee:  There is no fee for this service.
